Your research meets the federal criteria for exemption. You can fill out the IRB form and request exemption under question 4. You still need to fill out the rest of the form, since the IRB makes the final decision on whether a project is exempt or not. Therefore, the IRB needs enough information about the project to make that determination.
Please note that if you are working with a vulnerable population (next page) you cannot request exemption.
